Based on licensees own admission, the above regulation was not in compliance. Licensee has not conducted any disaster drills with the daycare children. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct an emergency disaster drill and submit proof of documentation to licensing no later than POC due date.

Based on record review and licensee own admission, the above regulation was not in compliance. 1 out of 3 children did not have immunizations on file. Licensee stated that parents do not wish to get the child immunized. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to either submit proof of immunizations, written medical exemption signed off by a physician, or a written plan of what she will do to come into compliance regarding C3 not having any immunizations. This is due to the department no later than the POC due date.

Based on record review, the licensee did not comply with the section cited above in 1 out of 3 children. C3 did not have a signed LIC995 on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit proof of LIC995 for C3 no later than POC due date.

Based on licensees own admission during record review, the licensee did not comply with the section cited above. Licensee has not been documenting 15 minute sleep checks. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Immediately, licensee agrees to begin implementing 15 minute sleep logs and submit to licensing no later than POC due date.
