Based on record review, the licensee did not comply with the section cited above in licensee has not conducted a fire drill since 9/12/24 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire drill and send a copy to LPA by 10/31/25. Send copy of current fire drill to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in infant in care missing 15 min sleep checks on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete 3 days worth of sleep checks and send to LPA by 10/31/25. Send 15 min sleep checks to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in licensee's mandated reporter certificate expired 9/25 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete mandated reporter training and send new certificate to LPA by 10/31/25. Send mandated reporter certificate to LPA at tammy.dutra@dss.ca.gov.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 files missing immunizations or updated immunizations for age of children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review the licensee did not comply with the section cited above in C2 was missing an LIC 9227, Individual infant sleep plan for a child in care under 12 months of age which poses/posed a potential health, safety or personal rights risk to persons in care.
