Based on observation, the licensee did not comply with the section cited above in shed was unlocked and poisons were accessible to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee locked shed during inspection. Poisons are no longer accessible. LPA advised to place a sign on the shed door to keep locked during childcare hours.

Based on record review, the licensee did not comply with the section cited above in assistant missing immunization record in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of MMR/TDAP for S2 by 3/21/25. Return Immunization record to LPA at tammy.dutra@dss.ca.gov.

Based on observation, the licensee did not comply with the section cited above, Infant C1 in crib with Boppy Pillow and blanket which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will watch safe sleep video provided by LPA Sims, Licensee will write a statement acknowledging licensee watched video and statement agreeing to not allow infants to sleep with any loose object in the crib and submit to LPA Sims by 2/29/24. Licensee will also have all parents with children currently enrolled sign LIC9224 by next day of attendance and all parents who enroll their children for the next 12 months sign LIC 9224 and maintain in child's file

Based on observationan and record review, the licensee did not comply with the section cited above, Licensee not documenting safe sleep checks for infant C1 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will watch safe sleep video provided by LPA Sims. Licensee will document 15 minute safe sleep checks for infant C1 and send copy of log to LPA Sims by 03/06/2024 to email sydney.sims@dss.ca.gov
