Resolution: Licensee will take the mandated reporter training and provide picture proof of the completed mandated reporter training. The licensee will email a scanned copy of the certificate and notfiy LPA of completion.
Based on observation, interview, record review, the licensee did not comply with the section cited above in 4 count out of 4 count object, persons, identifiers which poses/posed a potential health, safety or personal rights risk to persons in care.4 out of 6 children files were missing immunization records
Resolution: Licensee will contact all parents and redo all chidlren's files and obtain the required immunization record forms for each child. Licensee will also take a picture of the collected documents and send to LPA via email.

Infant is missing individual infant sleep plan in infant's file and 15 minute infant sleep checks were not being conducted.
Resolution: Licensee will notify infant's family to obtain the completed individual infant sleep plan. Licensee will also send a scanned copy of the completed individual infant sleep plan once received from family. Licensee will also begin conducting the 15 minute infant sleep checks and docuemnt. Licensee will send LPA a copy of the 15 minute sleep checks.
