Based on observation and interview, the licensee did not comply with the section cited above in 2 out of 9 persons were sleeping in bedroom with doors closed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During inspection Licensee opened bedroom door and stated doors will remain open and an asssistant will remain near to keep direct supervision of infants at all times.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 persons missing Mandated Reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update her Mandated Reporter training and have S3 complete hers and will submit proof to LPA via email.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 persons missing LIC9227 in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain LIC9227 from parents and provide proof to LPA via email.

Based on observation, the licensee did not comply with the section cited above in laundry soap, shampoos and bleach were accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: During inspection Licensee immediately moved products out of the reach of children and stated products will remain locked making them inaccessible to children in care.

Resolution: Licensee stated she will have staff renew mandated reporter and will provide proof to LPA vie email by 03/21/2023
