LPA met with Director Patricia Villicana, and provided purpose of inspection. At the time of inspection, LPA toured the facility, took census, interviewed and reviewed documents previously submitted to the department with Director. LPA interviewed 3 staff members during visit. Based on interviews conducted 3 of 3 staff members confirmed that Child #1 (C1) was left on the preschool playground without the supervision from their assigned teachers. Staff disclosed that the teachers transitioned inside the classroom but a name to face was not conducted, C1 was left outside on the enclosed playground for approximately 10 minutes until another teacher in passing noticed child unattended bringing them back to their assigned classroom. A Type A citation will be issued for Lack of Supervision and Civil Penalties in the amount of $500.00. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months. A Civil Penalty has been assessed on this visit. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. The Notice of Site Visit and Type A Deficiencies from today’s visit must be posted for 30 days.
