(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidence by: Based on interviews conducted and LPAs observation, staff failed to properly supervise C1 on two separate occasions, where C1 changed out of soiled clothing without staff's knowledge, and C1 had rubber wood chips in underwear from the playground, which poses a potential health, safety or personal rights risk to children in care.
