Based on record review, the licensee did not comply with the section cited above in C3 was missing an LIC 9227 in file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By POC date, Licensee agreess to submit proof of LIC 9227 for C3 and written statement of understanding of the regulation, to CCL.

Based on record review the licensee did not comply with the section cited above in C3-C6 were missing documentation of 15 minute sleep logs, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By POC date, Licensee agreess to submit proof of 15 minute sleep checks for C3 -C6 and written statement of understanding of the regulation, to CCL.

Based on observation, interview and record review, the licensee did not comply with the section cited above in when the Licensee was over capacity limits of the License at time of inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By POC date, Licensee agreess to submit a written statement of understanding of the regulation, to CCL.

Based on record review, the licensee did not comply with the section cited above in 4 out of 4 persons which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to start documenting the every 15 minute safe sleep checks for children up to 2 years old and submit proof to LPA via email. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on ecord review, the licensee did not comply with the section cited above in 1 out of 2 persons which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S2 complete the Mandated Reporter training and submit certifiate to LPA via email. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b) that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in fact the pediatric CPR/First Aid card was expired on 07/14/21, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Ms. Hughes agrees to attend a course for pediatric CPR/First Aid and submit a copy to Community Care Licensing by 12/24/21.
