BARAJAS, MELISSA FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above, when LPA observed the home to be at 58 degrees F and 7/7 children present complained of being cold, S2 was unable to turn on heat, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By POC date, Licensee agrees to abide by and provide CCL a written statement of understanding of the above regulation, ensuring the appropriate heating and ventilation for safety and comfort of children in care.

Based on observation, LPA observed 7 uncovered electrical outlets in the on limits area of the daycare facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By POC date, Licensee agrees to abide by the above regulation and provide CCL proof of the 7 electrical outlets to be covered during daycare operating hours.

Based on record review, the licensee did not comply with the section cited above in 1/10 childrens files reviewed (C6) , was missing the LIC 627, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By COB on POC date, Licensee agrees to submit proof of missing documentation to CCL.

Based on record review, the licensee did not comply with the section cited above in 7/10 childrens files reviewed did not have proof of immunization's. C4, C5, C6, C7, C8, C9 C10 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By COB on POC date, Licensee agrees to submit proof of the above missing documents, to CCL.

Based on record review, the licensee did not comply with the section cited above in 10/10 childrens files reviewed did not have evidence of an LIC 9150 on file. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By COB on POC date, Licensee agrees to submit proof of the signed LIC 9150 for children enrolled, to CCL.
