Based on observation, the licensee did not comply with the section cited above in 2 out of 2 infants were sleeping with a blanket in their crib which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall ensure that no infants are sleeping with anything in their cribs and send the LPA a photo of the infants sleeping with no blankets or any other objects. The photos shall be emailed to the LPA no later than 3/20/26.
Based on record review, the licensee did not comply with the section cited above in 3 out of 3 infants did not have an infant sleeping plan which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee shall have the parents complete the LIC9227, and if applicable, the Licensee shall ensure to fill out section "D" of the form. This shall be emailed to the LPA no later than 3/20/26.
Based on record review, the licensee did not comply with the section cited above as there was no immunization record for assistant Yuequan Yu, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email LPA a copy of assistant Yuequan Yu's immunization record by 5/13/2024.
Based on record review, the licensee did not comply with the section cited above as the CPR and First Aid certificate for assistant Yuequan Yu was not available which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall email LPA a copy of the CPR/first aid certificate or proof of registration by 5/13/2024.

Based on observation, the licensee did not comply with the section cited above in 3 out of 3 infants which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete the sleep log for infants in care for at least 5 days in a row and email a copy to LPA by 05/13/2024.
