Based on observation and interview, the licensee did not comply with the section cited by operating out of ratio which poses/posed a potential health, safety or personal rights risk to persons in care. There should be a ratio of 1 staff to 8 children for Title 5 facilities. Another staff did arrive during the visit and the facility came back into compliance.

Type B citation - potential risk if not corrected

Based on observation and interview, the licensee did not comply with the section cited by operating out of ratio which poses/posed a potential health, safety or personal rights risk to persons in care. There should be a ratio of 1 staff to 8 children for Title 5 facilities. Another staff did arrive during the visit and the facility came back into compliance.

Licensee shall maintain a lead value at or below the Action Level of 5.5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care. This requirement is not met as evidence by: Based on record review the licensee did not comply with the section cited above as outlet Bubbler- Room P1 Sink exceeded the acceptable amount of lead allowed in a child care center, which poses a potential Health and Safety risk to persons in care.

(b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s).
