Based on observation the licensee did not comply with the section cited above in that there is a hot tub in the back yard with a cover that does not properly lock which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit by close of business on 6/27/2023 a written plan of correction of when and how this deficiency will be corrected. Licensee has agreed to keep backyard off limits to children in care until correction is complete.
Based on (record review), the licensee did not comply with the section cited above in that mandated reporter training is expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall submit proof of completion of the mandated reporter training to licensing by POC date.
