A REVIEW OF RECORDS REVEALED 3 CHILDREN DO NOT HAVE A PHYSICIAN'S REPORT IN FILE.
Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: To be accorded dignity in his/her personal relationships with staff and other persons. This requirement has not been met as evidenced by: Based on interviews and an incident report, a child's personal rights were violated which is an immediate safety risk to children in care.
interviews which revealed that a substitute teacher was left to supervise children on the playground, but was not notified of a child's behaviors prior, leading to another child being injured.
.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement has not been met as evidenced by the lead testing results received, indicating that two outlets in the facility had results in exceedance of the 5 ppb Action Level. This poses a potential risk to the children in care.
