Based on interview and record review, the licensee did not comply with the section cited above as Licensee did not have acitve CPR/ FA certification which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will take CPR/ FA class and will sned a picture od the certificate obtained by the date above to LPA via email or text cynthia.alvarez@dss.ca.gov or 805-689-4212
Based on interview and record review, the licensee did not comply with the section cited above as they did not have fire extinguisher that had been purchased withing the last 12 months which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send a picture of the receipt of the fire extinguisher once it is delivered to LPA by the date stated above
Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 5 records reviewed did not have current immunization records or did not contain an immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will contact parents to obtain current immunization records for the children and will send a copy of the record to the LPA by the dates listed above
Based on observation and interview , the licensee did not comply with the section cited above, fire exinguisher has not been serviced or newly purchased within the year which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will purchase a new regulation fire extinguisher and provide proof to LPA. LPA was provided a purchase reciept for order of new fire exinguisher.
Based on observation, interview and record review, the licensee did not comply with the section cited above as the children's roster was not updated which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide an emailed copy of current and updated roster by email.
