Based on interview and record review, the licensee did not comply with the section cited above. Licensee's adult son, David Sinohui who resides in the home does not have a criminal record clearance and has not attempted to obtain a clearance since turning 18 in 2021 which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will have David Sinohui obtain a livescan and will send proof to LPA via picture.
Based on record review, the licensee did not comply with the section cited above in 2 out of 2 children who did not have a signed LIC 995A on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain a signed LIC 995A for Child 2 and Child 3 and will provide proof to LPA via picture.
Based on observation, the licensee did not comply with the section cited above. LPA did not observe a carbon monoxide detector in the facility. Per Licensee, the smoke detector is also a carbon monoxide detector but LPA could not verify this which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain an operable carbon monoxide detector and will provide proof to LPA via picture and video.
Based on interview, the licensee did not comply with the section cited above. Licensee stated she does not have proof of Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete the Mandated Reporter training available on www.mandatedreporterca.com and will complete the general and the child care provider training. Licensee will send proof of the certificate of completion to LPA via picture.
