Based on observation, the licensee did not comply with the section cited above in 2 out of 2 fire extinguishers was last serviced on 2/14/24 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will have fire extinguisher serviced or purchased new and send service tag or purchase receipt as proof of correction by agreed upon date.

Based on record review, the licensee did not comply with the section cited above in licensee could not provide proof of Mandated Reporter trainin during inspection visit which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states they will complete Mandated Reporter Training and submit certificate to LPA via email as proof of correction by agreed upon date.

Based on observation and record review, the licensee did not comply with the section cited above licensee did not have a current diaster drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conducted a diaster drill log and log all drills and submit to LPA as proof of correction by agreed upon date.

Based on record review, the licensee did not comply with the section cited above in 2 of 2 staff had expired Mandated Reporter Training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant wil complete Mandated report training and submit certificates to LPA as proof of correction by agreed upon date.

Based on record review, the licensee did not comply with the section cited above. Adult 4/ Assistant did not have mandated reporter certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have adult 4 complete the mandated reporter training, general and child care provider course and will submit proof to LPA via email. LPA advised training can be completed on www.mandatedreporterca.com

Based on Record review, the licensee did not comply with the section cited above. Adult 4 did not have proof of required immunizations for Measles, pertussis and Flu on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain proof of required immunizations for adult 4 and will submit proof to LPA via email.

Based on interview and record review the licensee did not comply with the section cited above in 1 out of 4 adults present who did not appear on the facility roster which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC 9182, LIC 508 and a copy of an ID to the Regional Office to have the transfer request for Adult 4, Aracely Ferrer, completed.

Based on record review, the licensee did not comply with the section cited above. adult 4 did not have proof of requried immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain proof of required immunizations for adult 4 and will submit proof to LPA via email.
