Based on observation, the licensee did not comply with the section cited above. The 2A10BC fire extinguisher had a service tag which was dated 9/2020 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have fire extinguisher serviced or will purchase a new 2A10BC fire extinghuisher. Licensee will provide proof of service tag to current fire extinguisher or receipt to new extinguisher via picture.
Based on record review, the licensee did not comply with the section cited above. Licensee did not have proof of immunization for pertussis which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain proof of immunization to pertussis and will submit proof to LPA via picture.

Resolution: Licensee stated that she will enroll in an American Red Cross Pediatric CPR/First Aid course and provide LPA a copy of course taken by POC date of 10/08/2021 to LPA via email.
