Based on observation, the licensee did not comply with the section cited above in LPA did not observe pool cover or pool alarm at time of inspection which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per the licensee, the alarm purchased was returned because do to malfunctioning sensors. Licensee states she will purchase a pool cover to met regulation. LPA will conduct POC visit.
Based on observation, the licensee did not comply with the section cited above in 2 out of 2 required safety equipment was not observed at facility. LPA did not observe life ring or rescue pole which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per the licensee, safety equipment was ordered at time of inspection. LPA observed order confirmation for life ring and rescue pole. LPA will conduct POC visit.
Based on observation and record review, the licensee did not comply with the section cited above in 2 out of 2 staff had expired mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states staff will complete Mandated Reporter training with union representative on 2/26/26 and submit certificates to LPA as proof of correction. LPA will conduct POC visit
