Based on observation, interview, record review, the licensee did not comply with the section cited above in that animal feces was found along the back fence of the play yard which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that LPM and LPA observed 42 children present the addtional children were from facility # 197493615 which poses an immediate health, safety or personal rights risk to persons in care.

The licensee did not comply with the section cited above in that there were 42 children in care that exceeded the facility fire clearance which poses an immediate health, safety, or personal rights risk to persons in care.

The licensee did not comply with the section cited above in that Director on 06/18/25 did not allow inspection of documents at the facility by Departments representatives which poses a potential health, safety, or personal rights risk to persons in care.

(c) The Department has authority to inspect, audit, and copy child and child care center records upon demand during normal business hours. Recrods may be removed if necessary for copying. Removal of records shall be subject to the requirements in Sections 101217(c) and 101221(d).

(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.

(d) All personnel records shall be maintained at the child care center and shall be available to the licensing agency for review.

(d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.
