Based on record review, the licensee did not have the completed LIC9227 in the infant's file during inspection. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will email the LIC9227 by 03/11/2025 for current infant enrolled that is under 12 months.
Based on observation and record review, the licensee did not comply with the section cited above. There was no documentation or log available by the Licensee for the time of each 15 minute check for the three infants present, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will generate a log for the infants enrolled and document the time of each 15 minute check and provide a copy of the log to CCL by the POC due date.
