Upon arrival, LPA met with director, Brenda Shaw. A COVID-19 risk assessment was conducted. LPA provided Director with a copy of the LIC 125 Entrance Checklist to facilitate the inspection. Business hours are from Monday to Friday, 9:00 AM to 4:00 PM. The entrance to the facility is locked and parents enter using a call/buzzer system. At around 9:20 am, LPA was guided to a facility tour by the director. All areas identified on the Facility Sketch were inspected. The facility has a total of two classrooms ( Classroom A 114 and Classroom B 115). Facility was observed to have cameras indoors and outdoors with no sound available. LPA observed facility to be equipped to isolate and care for any child who becomes ill during the day. The isolation area was observed in a designated isolation room next to the director office. The isolation area was observed to have a cot for each ill child. LPA observed the following children and staff present during inspection: Classroom A 114: five children / three staff; Classroom B 115: no children. Teacher child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under visual supervision of a teacher. The facility was observed to be within the license capacity and limitations. The facility roster was up to date and emergency disaster drills were conducted on during February 2023. LPA reminded the Facility Representative that Fire/Disaster Drill should be conducted every 6 months. Emergency disaster plan is updated and reflected to the current year. The number of children present matched the number of children signed-in. Licensing documents. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed. LPA observed the following postings located in the entrance way: License LIC203A, LIC 613A, PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems, and a contingency plan for action during fires, floods and earthquakes. Menus were observed to be in writing and posted to be review by the child's authorized representative. LPA observed working carbon monoxide detectors in the facility. LPA observed a working telephone service on the premises. Cots used for napping were observed to be maintained in a safe condition. Cots are wiped with a detergent/disinfectant weekly or when soiled or wet. Sheets are provided to cover the cots and blankets for each child. Facility launders the sheets and blankets. The licensee provides a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities. All toilets and hand-washing areas were observed to be maintained in safe and sanitary operating condition. Drinking water from a non-contaminating fixture was observed to be readily available both indoors and in the outdoor activity area. Per Director City of Los Angeles Department of Recreation and Parks Forestry Division assists in measures to keep the center free of flies, other insects, and rodents. All containers used for storage of solid wastes, including moveable bins, were observed to have a tight-fitting cover that is kept on; in good repair; and leakproof and rodent-proof.
