Resolution: By plan of correction due date of 9/26/25, Licensee will submit proof of 15 minute sleep checks for the two infants in care. Documentation shall include the date, name of the child, time of check, initials of person who checked, and notate any signs of distress.
Based on record review of children's files, the licensee did not comply with the section cited above in 1 out of 1 infants under 12 months old which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By plan of correction due date 9/26/25, Licensee will submit LIC 9227 for Child 3.
