Based on observation, interview, record review, the licensee did not comply with the section cited above. In the backyard the licensee had a roll of artificial grass with several nails sticking out of the corners and loose cement parking stops accessible to children during outdoor play which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Per licensee they will remove the artificial grass roll and cement parking stops from the backyard and postpone outdoor play until these items are removed and provide pictures by the POC due date.
Based on observation, interview, record review, the licensee did not comply with the section cited above. C1 and C2 do not have their completed Individual Sleep Plan on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee they will complete the infant sleep plan for infants in care by the POC due date.
