Based on observation, the licensee did not comply with the section cited above, there were loose articles in the portable play yard which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed items from the portable play yard at the time of inspection, bringing the facility back into complyance at the time of inspection. LPA reviewed safe sleep regulation with Licensee.
Based on record review, the licensee did not comply with the section cited above, Licensee's assitants did not have Mandated Reporter certificates in record file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will work with her assistants to complete the training and send the certificates to LPA by Plan of correction date.
Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Licensee did not ensure to have all of the children's files complete. Some files were missing form LIC995A.
Resolution: Licensee will submit one complete child's file to LPA by POC due date.
