Based on record review , the licensee did not comply with the section cited above, 1 out of 7 children has no record of imunization on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agreed to submit a Plan of Correction (POC) letter to Community Care Licensing (CCL) no later than November 14, 2025. The POC shall outline the measures to ensure that, prior to admission, all children are immunized against communicable diseases.

Based on observation, the licensee did not comply with the section cited above, fire extinguisher was last serviced on 3/19/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agreed to submit a Plan of Correction (POC) outlining procedures to ensure that fire extinguisher is serviced annually or replaced with a new purchase every year. The POC letter, along with proof of service or proof of purchase, shall be submitted to Community Care Licensing no later than November 14, 2025

Based on record review, the licensee did not comply with the section cited above, Assistant does not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit the immunization record of assistant and submit a written plan of correction outlining the measure to prevent the recurrence of the similar violation.

Based on record review the licensee did not comply with the section cited above, Licensee mandated reporter training certificate expired on 7/7/2024 and assistant has not taken the said training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed tosubmit proof of training and submit a written plan of correction outlining the measures to prevent the recurrence of the same violation no later than 8/12/2024.

Based on record review, the licensee did not comply with the section cited above, Assistant does not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete and document the the immunization record in PM 286 submit a written plan of correction outlining the measure to prevent the recurrence of the similar violation. POC shall be submitted no later than 8/12/2024.

Based on record review, the licensee did not comply with the section cited above, the last time FCCH conducted the drill was in 1/18/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to conduct the Fire and Disaster drill te next business day, when day care children are present and submit a written plan of correction outlining the measures to prevent the recurrence of the same violation no later than 8/12/2024.

Based on record review, the licensee did not comply with the section cited above, Infant # 2 does not have Infant Safe Sleep Plan (LIC 9227) on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to obtain parents signature for LIC 9227 and moving forward licensee understood that Individual Safe Sleep Plan is required for Infant 0-12 months old. Written plan of correction will be submitted to CCL no later than 7/7/2022.

Based on record review, the licensee did not comply with the section cited above, Licensee and Staff did not take/renew the Mandated reporter training per AB 1207 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant agreed to take Mandated Reporter Trainng per AB 1207 and submit proof to Community Care Licensing (CCL) no later than 7/7/2022 to
