Based on observation , the licensee did not comply with the section cited above in LPA obseved 2 infants ( one napping and one was awake) covered with blanket in a carseat which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed infants covered with blankets from the car seat. Licensee agreed to submit a plan of correction outlining the measure to ensure that infants will not be covered with blanket at all times. POC due date is 9/30/2024.
Based on observation, the licensee did not comply with the section cited above 2 infants, Infant 1 and Infant 2 were observed in a car seat covered with blankets which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee immediate removed the infants from the car seat carrying one and placing the other in the gated play yard placed Licensee agreed to submit a plan of correction outlining measures to ensure that car seat shall not be used for sleeping but shalbe used for transportation only. POC is due on 9/30/2024
Based on observation, the licensee did not comply with the section cited above, upon LPA's arrival at the FCCH, LPA observed four (4) infants, and five (5) children over 2 years old were present under the care of licensee alone which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: At 9:27 AM, Assistant Mirna Leon came making the facility within ratio. Licensee will submit a plan of correction outlining measures how to maintain the staffing ratio at all times. POC shall be submitted no later than 9/30/2024.
Based on observation, the licensee did not comply with the section cited above, Emergency Disaster Plan is not posted and cannot be found which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall comlete and post the Emergency Disaster plan. LPA provided the form to LPA PDC is due by 10/7/2024.
Based on interview and record review,the licensee did not comply with the section cited above Licensee stated that she cannot find the fire and disaster log which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction on how to ensure that fire and disaster drills shall be documented. POC shall be submitted no later thatn 10/7/2024.
Based on record review, the licensee did not comply with the section cited above, infant sleep log is not updated which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to document napping infant every 15 minutes and shall submit a plan of correction outlining measures on how to complete the sleep log. POC is due on 9/30/2024.
Based on record review the licensee did not comply with the section cited above, Assistant Mirna Leon does not have immunization record on file which poses/posed a potential health, safety or persona rights risk to persons in care.
Resolution: Licensee agreed to submit the required immunization record of Ms. Leon to CCL no later thatn 10/7/2024
Based on observation and interview, the licensee did not comply with the section cited above, LPA observed that a wall was constructed in the garage in between a pre- existing room to create an additional room being used by children to play. Licensee stated that the unpermitted construction was performed in 9/2022, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit an LIC 999A Facility Sketch with measurement of all the roomsa and spaces in the home labeling the accessible and the off limit areas. POC is due on 10/7/2027
Based on observation, the licensee did not comply with the section cited above the created room and the supposed off limit room are beig used by day care children which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction no later than 10/7/2024, licensee further agreed not to utilize the off-mits room without submitting the updated LIC 279 - application form, LIC 999A - facility sketch, fire safety inspection clearance and prior to CCL inspection.
Based on record review, the licensee did not comply with the section cited above Infant 1 and 3 who were enrolled before 12 months old did not have Individual the Safe Sleep Plan which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to complete the LIC 9227 with the help of the parent for all infants less that 12 month old at the time of enrollment. POC letter shall be submitted no later than 10/7/2024.
Based on observation, the licensee did not comply with the section cited above infants 3 and 4 were observed in a crib while awake which poses/posed a pote ntial health, safety or personal rights risk to persons in care.
Resolution: As soon as the assistant arrived at the facility, the assistant picked up the infant 3 and infant 4 carrying one and placing the other on the matted floor. Licensee agreed to submit POC letter outlining measures how to ensure that infants that are awake shall not be placed in a crib or pack and play.
Based on record review, the licensee did not comply with the section cited above Licensee and assistant did not renew the Mandated Reporter Training per AB 1207 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant agreed to submit the certificate to CCL no later than 10/7/2024
