Based on interview with Director and record review, the licensee did not comply with the section cited above as 2 out of 2 outlets have not been tested for lead exceedance. Direcror was unable to provide LPA with test results of outlets in Room #Y1 which poses a potential health, safety or personal rights risk to persons in care.
Based on the information gathered from the unusual incident report, interviews with two staff, and review statements from the four staff. The children were in the bathroom together for approximately five minutes without the supervision. This is an immediately safety risk to children in care.
