Based on interview and record review, licensee Mandated Reporter training expired on 11/04/2024 and Staff #2 (S2) Mandated Reporter training expired on 04/21/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email proof of completion of Mandated Reporter training for licensee and S2 to LPA Cha by POC due date.

Based on observation and interview, the in-ground pool and jacuzzi in off-limits backyard area did not comply with Title 22 regulations, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide an approved pool safety cover or alarm that meets ASTM approved standard F 1346-23.Licensee will provide US Coast Guard approved life ring, 12 foot fixed rescue pole, and daily pool inspection documentation. Licensee will reinstall key lockable device to be at least 60 inches from the ground, reinstall gate to be self-closing and self-latching, and vertical clearance from the ground to be no more than two inches, and email proof to LPA Cha by POC due date.

Based on observation, interview, and record review the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Two out of three staff did not have the complete required immunizations on file: MMR, TB and Tdap.

Resolution: Licensee will submit a copy of both two staff's immunization Record with TB, MMR Tdap by text or email by the indicated due date.

Resolution: Licensee will begin to complete the 15 minute check, log in time and submit a copy to the Pamdale Regional office by due date by email, via mail or text,

Resolution: Licensee will begin to complete the indiviual infant sleeping plan LIC 9227 and submit a copy to the Pamdale Regional office by due date by email, via mail or text,

Based on LPA observation, the License was providing care to nine children which exceeds the staffing ratio and capacity for a small family child care home. This poses an immediate Health, Safety or personal right risk to the children in care.

Resolution: LPAs observed assistant arrive at facility within 30 minutes.

LPAs observed an uncleared adult living in the premises, in the off limits areas. This poses an immediate health and safety risk to children in care. A type A citation will be assessed, as well as an immediate civil penalty of $500 will be assessed

Resolution: LIicensee will provide proof of uncleared adult by no later than 11/03/2022
