Resolution: Licensee will contact parents and request immunization records for children and mail, email a copy to the Palmdale Regional office by date specified
Based on observation, interview, and record review, the licensee did not comply with the section cited above infant under one year of age did not have the LIC9227 completed by parent nor Licensee in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide parent the LIC9227 and submit completed copy to the Palmdale Regional center by email, or text by the date indicated above.
Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee and staff was not able to provide the Child Care provider(AB1207) certifcate which poses a potential health, safety or personal rights risk to persons in care.
