Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During Record review LPA observed that licensee and assistant CPR/first aid expired on 1/21/25. Licensee stated that they were planning to schedule a training and Licensee had health issue.

Resolution: Licensee agreed to send completed CPR/First aid to LPA via email by end of day Wednesday 4/9/25.

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During record review LPA observed licensee and assistants Mandated reporter expired on 3/15/24 and 1/16/25. Licensee stated that they assumed it need to be done one time.

Resolution: Licensee agreed to send complated mandated reporter training certifiates to LPA via email by end of day Wednesday 4/9/25.

Based on observation and record review the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed that the licensee did not have a vaild mandated reporter training on file. Assistant stated that it was missed placed and was not able to find it.

Resolution: Assistant agreed to send Vaild Mandated reporter training certificate to LPA via Email or text by end of day Thursday 1/18/24

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed S1 did not have immunization in S1 folder, S1 has contacted hospital for copy of immunization.

Resolution: Licensee agreed to send copy of S1 immunization to LPA Via email or text by end of day Thursday (1/26/23) Licensee also agreed to send a letter to LPA stating how Licensee will make sure that this won't happen in the future.

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed that S2 did not have a current mandated reporter training . Licensee was not able to provide a current mandated reporter training to LAP

Licensee stated that, licensee might have lost it or have not received it back from the parents.

Resolution: Licensee agreed to send signed personal right to LPA via email or text by end of day Tuesday (1/17/23) Licensee also agreed to send a letter to LPA stating how Licensee will make sure that this won't happen in the future.

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.LPA reviewed childrens file and observed that there are multiple children did not have a sign personal rights. .Licensee stated that, licensee might have lost it or have not received it back from the parents.

Based on record review conducted by LPA, it was determined that immunization records (pertussis, measles, and influenza) for assistant provider were not obtained by Licensee. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall obtain immunization records for pertussis, measles, and influenza by the POC due date and submit to LPA.

Based on record review conducted by LPA, immunization records for children in care C4, C7, and C11 are not available for review by LPA. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall obtain immunization records for children C4, C7, and C11 and submit copies of all to LPA before the due date.

Based on record review conducted by LPA, it was determined that neither Licensee nor assistant present during today's visit possessed a valid Mandated Reporter training certificate. Licensee stated that she has not completed the renewal and that assistant has never taken the training. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall ensure Mandated Reporter trainings, General and Child Care Providers, are completed for all care providers in the home. Licensee shall submit requested documents to LPA before the POC due date.

Based on record review conducted by LPA, it was determined that no adults present in the home had valid CPR/1st Aid certification required by the above cited regulation. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall enroll in EMSA-certified pediatric CPR/1st Aid course and submit proof of enrollment to LPA. Licensee shall submit proof of completion to LPA when it is obtained.
