Based on interview & record review, the licensee did not comply with the section cited above as the licensee has not performed/documented an emergency drill every 6 months as required which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create a written plan for how the facility will conduct and document emergency drills every 6 months. This written plan should prove that the licensee understands and acknowledges this requirement will be met moving forward. Licensee will email this written plan to LPA Uribe at christina.uribe@dss.ca.gov no later than the due date of 10/10/2025.

Based on record review, the licensee did not comply with the section cited above as the licensee and two assistants either have expired or incomplete certifications for mandated reporter trainings which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and two assistants will go to www.mandatedreporterca.com and take the Mandated Reporter for Child Care Providers course and obtian a new certificarte. Licensee will email a copy of all mandated reporter certificates to LPA Uribe at christina.uribe@dss.ca.gov no later than the due date of 10/10/2025.

Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.LPA observed that the fire drill was conducted on 3/24/23. Licensee stated that they forgot to do a drill with in 6 months

Resolution: Licensee agreed to send an updated fire drill to LPA via email or text by end of day Friday 12/22/23.

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA reviewed assistants file and observed no immunization. Licensee stated that assistant has it but did not give a copy to licensee.

Resolution: Licensee agreed to send assistant immunization to LPA via email or text by end of day Wednesday 12/27/23

Based on observation and record review, the licensee did not comply with the section cited above as all 5 children files did not have immunizations, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee would maintain and document immunizations on California blue immunization form by 07/26/2022 Licensee would send proof to LPA by 07/26/2022
