Based on LPA observation, the licensee did not comply with the section cited above. LPA observed that dish soaps and cleaning compounds were accessible in the kitchen and bathroom. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee fixed broken child locks in the kitchen and placed cleaning compounds in the bathroom in a location inaccesible to children.
Based on record review, the licensee did not comply with the section cited above. Based on record review, neither Licensee not assistant provider had completed the Mandated Reporter training. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall obtain Mandated Reporter training certificates for Licensee and assistant provider by the POC due date and submit proof of completion to LPA.
