Based on interview and record review, LPA determined that from 9:20am to 9:45am, 10 children were left under the supervision by 2 aides, neither of whom had ECE units, and who were therefore unqualified to count as teachers in ratio. This poses an immediate health, safety, or personal rights risk to persons in care.

the children's files being incomplete during inspection. The Facility is in the process of digitizing files. When reviewing digital files, forms where found to be without signatures and/or overall incomplete. Physical files were also reviewed and found to be missing required documents such as the LIC 613A and physician signed immunization exemption forms. This is a potential health and safety risk to children in care.

Limitations on Capacity (a): A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidence by the Director self reporting an incident where the child noted in the report was under the age that their license is designated for.
