Based on staff record review which revealed L1, S2, and S3 did not have a current AB1207 mandated reporter training. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that L1, S2, and S3 will complete the mandated reporter training (MRT) by 12/24/2025 and submit a copy of the MRT certificate to jennifer.patel@dss.ca.gov. Licensee stated her and all staff will completed MRT every two years subsequently.

Based on LPA's staff record review, S2 records were missing evidence of immunity against measles, pertussis and Tuberculosis. Additionally, LPA's staff record review indicated S3 records were missing evidence of immunity against pertussis and Tuberculosis. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide S2s evidence of immunity against measles, pertussis and Tuberculosis and S3s evidence of immunity against pertussis and Tuberculosis and submit proof to jennifer.patel@dss.ca.gov by 12/24/2025.

Based on observation and interview, the licensee stated she was not aware that sleep logs were required for children over 12 months old and was unable to provide 15 minute sleep logs for C4 and C5. The licensee did not comply with the section cited above for two out of two infant files reviewed during today's inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee started 15 minute sleep logs for C4 and C5 while LPA was present and will continue logs until C4 and C5 turn 24 months old. Licensee agreed to continue safe sleep logs for all infants, ages 0-24 months in care. Licensee will email completed copies of sleep logs 11/24/25 to 12/10/25 for C4 and C5 to jennifer.patel@dss.ca.gov.
