Based on staff record review which revealed S2 S2 did not have a current AB1207 mandated reporter training. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that S2 is currently in the process of completing the mandated reporter training (MRT) and will ensure that S2 completes MRT by the POC due date, 10/1/2025 and submit a copy of the MRT certificate to jennifer.patel@dss.ca.gov.

Based on children's records reviewed which revealed C1-C6 were either missing immunization records (IR) or the IR was not up to date. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LPA evidence of updated immunization records for C1-C6 to jennifer.patel@dss.ca.gov by 10/1/2025.

Based on children's records reviewed which revealed C1-C6 were either missing CDPH 286 or the CDPH 286 was incomplete. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LPA evidence that the CDPH 286 for was completed for C1-C6 to jennifer.patel@dss.ca.gov by 10/1/2025.

Based on the LPA's observation of the facility's fire drill log, the last fire drill documented was on 8/8/2024. Licensee confirmed the last fire drill was completed in February 2025, but was not documented. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete a fire drill, document the date on the fire drill log and submit a copy of the updated log by 10/1/2025 to jennifer.patel@dss.ca.gov.

Based on LPA's staff record review, S1 records was missing evidence of immunity against measles. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provided S1's evidence of immunity against measles and submit proof to jennifer.patel@dss.ca.gov by 10/1/2025.

Based on children's records reviewed the Licensee was unable to furnish proof that 15 minutes checks were being conducted for C3 - C5. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she was using an online application to log sleep checks, but the system reset and she did not have access to the 15 minute checks any longer. Licensee stated she should have wrote them down on paper and she is going to use a paper chart moving forward. Licensee stated she will start the 15 minute checks immediately and will send to LPA at melinda.mohr@dss.ca.gov

Based on children's records reviewed it revealed C1-C5s immunization records were not transcribed onto the blue CDPH 286 form. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee stated that she will transfer the children's immunization records onto the blue CDPH 286. LIcensee will email proof to LPA @ melinda.mohr@dss.ca.gov.
