Based on interview and record review, the licensee did not comply with the section cited above in which licensee did not document 15 minute sleep checks for C4, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will start documenting the 15 minute sleep checks today 04/10/2025 and will send copies to LPA at Melinda.mohr@dss.ca.gov
Based on interview and record review, the licensee did not comply with the section cited above in which both L2 and S1 did not have current mandated reporter training certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have both L2 and S1 take the training and will send the certificates to LPA at Melinda.mohr@dss.ca.gov
