Based on record review, the licensee did not comply with the section cited above no staff present had a valid first aid/cpr certification which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will have all of their staff take and complete the first aid class and obtain certification. Proof will be provided to LPA Peebles by COB

Based on record review, the licensee did not comply with the section cited above C2 did not have a completed signed LIC 627 consent for emergency medical treatment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they have ghave C2's guardian complete the LIC627 t end of day during pick up on 05/28/2025. Proof will be provided to LPA by photograps.

Based on record review, the licensee did not comply with the section cited above in which child #5 does not have form LIC 995A (Parent's Rights) in his/her file available for review during this inspection. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Orzine Venable agrees to obtain a copy of the form LIC 995A (Parent's Rights) and submit a copy to Community Care Licensing by 06/22/23. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b, that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on observation, the licensee did not comply with the section cited above in which there are 16 childen present and the licensee is licensed for a maximum of 14 children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee, Orzine Venable agrees to submit in writing or email on how she will remain in compliance with Title 22 Regulations on her licensed capacity. Licensee was reminded that per California Health and Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b, that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on record review, the licensee did not comply with the section cited above in which havestaff #2 & #3 do not have form LIC 9052 (Notice of Employee Rights) available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Orzine Venable agrees to obtain a copy of the form LIC 9502 (Notice of Employee Rights) for staff #2 & #3 and submit copies to Community Care Licensing by 06/22/23. Licensee was reminded that per California Health & Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b), or 1597.58(b, that there would be a $100.00 Civil Penalty per day for every day the plan of correction is late.

Based on observation, the licensee did not comply with the section cited above in which there were 16 children present,with three of the children being infants an zero school age children. The licensee was alone until an assistant arrived 37 minutes later which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in which staff members #1 & #2 do not have a current Mandated Reporter Certificate available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.
