VALDES FAMILY CHILD CARE

Based on observation and record review, two adults residents were still in process according to Guardian background system, therefore the licensee did not comply with the section cited above in 2 out 4 persons living in the home which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have two adult residents re-fingerprinted and will send evidence to LPA via email or text by 08/29/25. Licensee will ensure residents are not present during childcare hours until background check is fully cleared. Licensee will send clearances to LPA via email or text once received.

Based on interview and record review, Licensee and assistant did not have valid mandated reporter training completed, therefore the licensee did not comply with the section cited above in 2 out of 2 staff reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit completed CHILDCARE PROVIDER mandated reporter training certificates to LPA via email by 09/05/25.

Based on observation, the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee called C1,C2, and C3 parents to come pick their children up from the facility due to being over ratio. LPA reminded Licensee unless she has an assistant she is to operate as a small family child care.

Based on LPA S.Brewer's record review, the licensee Dalia Valdes, did not comply with the section cited above and failed to provide proof of Mandated Reporter Certificates for General and Child Care Provider Training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee Dalia Valdes, agrees to complete the Mandated Child Abuse General and Child Care Provider Training and submit proof of Training certificates to the department on or before 09/06/02022.

Based on LPA S.Brewer record review on 09/01/2022, the licensee Dalia Valdes, did not comply with the section cited above in 4 children out of 7 children present, identified as Children #1, #5, #6 and #7 did not have the LIC627 Medical Consent forms on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee Dalia Valdes, agrees to obtain the missing LIC627 Medical Consent form for all current enrolled children and ensure the the forms are completed for future enrolled children. The licensee also agrees to submit proof of the consent forms for current enrolled children to the department on or before 09/02/2022.
