Based on interview and record review, no current child roster was available at the time of inspection, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send LPA proof of a current roster via email or text by 03/13/26
Based on interview and record review, C1 and C3 did not have the required immunizations at the time of enrollment and do not currently have the amount required, therefore the licensee did not comply with the section cited above in 2 out of 3 children's records reviewed, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have parents of C1 & C3 submit the most updated immunization records and will have a medical professional provide a schedule for the remaining immunizations needed for their age. Licensee will submit the records and schedules to LPA via email.
Based on observation, the licensee did not comply with the section cited above LPA observed C1 asleep in a packnplay while laying on a boppie which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated C1 has a neck issue so parents requested C1 sleep on a boppie, however Licensee will inform parents that due to Title 22 regulations infants are not allowed to sleep with any additional items in the crib or packnplay. Boppie was removed while LPA was present.
Based on record review, the licensee did not comply with the section cited above S2 did not have completion of mandated reporter certification on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure S2 completes mandted reporter training ad obtain a completed certificate and will provide proof to LPA via email by 02/28/2024.
Based on record review, the licensee did not comply with the section cited above S2 did not have any documentation of immunizations which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obatin a copy of S2 immunizations and a copy will be provided to LPA via email by 02/28/2024
