MARTIN FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above C1 did not have LIC627 consent to emergency medical treatment in their files, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have parent sign LIC627 medical consent form. Licensee obtain a signed copy for the child's records and submit the signed copy to LPA via email by 03/29/24.

Based on LPA's observation, the licensee did not comply with the section cited above in 2 children were in high chairs and not eating watching television. Which poses an immediate health, safety or personal rights risk to persons in care. Child 1 was put in high chair so "he doesnt make a rukus" while the other children sleep. Child 2 fell asleep after eating and watching TV and was not moved out of high chair.

Resolution: Licensee agrees to be in compliance with Title 22 regulations pertaining to personal rights by 03/28/2023. LIcensee will submit written documentation agreeing to not use high chairs for anything other then feeding. Licensee was reminded of the $100.00 Civil Penalty for every day the POC is late, per California Health & Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b) or 1597.58(b).

Based on record review, the licensee did not comply with the section cited above which S4 was missing immunization records, mandated reporter which was not available for review during inspection. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agree's to submit proof of immunization and mandated reporter certificate for S4. Licensee was reminded of the $100.00 Civil Penalty for every day the POC is late, per California Health & Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b) or 1597.58(b).

Based on record reviews, the licensee did not comply with the section cited above in that staff #2 does not have proof of immmunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, Edna Martin agrees to submit immunization records for staff #1 by 02/14/22. (MMR/TDAP and Influenza shot) or a written declaration for the influenza shot.
