Based on record review, the licensee did not comply with the section cited above S1 and S2 did not have a valid First aid/CPR certification which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated boith S1 and S2 will take and complete a CPR/ First aid training an obtain certification that will be submitted to LPA by COB 11/07/2025
Based on record review, the licensee did not comply with the section cited above S1 did not have TB,MMR on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will ensure S2 obtains documentation of the required immunizations. Proof will be submitted to LPA by COB 11/07/2025
Based on record review, the licensee did not comply with the section cited above several children did not have any immunizations and the request for immunization exemption was signed on the back of the blue cards which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee understands that all children enrolled shall have all the required immunizations. If they do not have immunizations children must have substantial reasoning for not obtaining immunizations. Proof of disenrollment or substantial documentation must be provided to LPA by COB 11/07/2025.
Based on record review, the licensee did not comply with the section cited above S1 did not have a mandated reporter certification on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure S1 takes and completes the mandated reporter training course. Proof of valid certification will be provided to LPA via text message.
Based on record review, the licensee did not comply with the section cited above S1 did not have immunizations readily available for viewing which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of S1's immunizations to LPA via text by 11/19/2024
Based on Record review licensee did not comply with the regulation above. During record review LPA observed that last time a fire and disaster drill was done was in 2022. This poses a potential health and safety risk to children in care. A citation will be issued.
Resolution: Licensee stated she will conduct a fire and disaster drill and document the drill. Proof will be provided to LPA via text by 01/04/2023.
