Based on observation and interview, the latching mechanism on the mesh gate did not close properly when self closing, therefore the licensee did not comply with the section cited above which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit plan for correction, including date of work to be done and company info prior to work being done. Once finished, Licensee will submit photographic and video evidence once latch is working properly when self-closing by POC date of 01/29/25. Backyard will remain off-limits until correction is made.
