Based on interview and record review, the licensee did not comply with the section cited above in 1 of 1 infant did not have sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will start completing sleep logs for up to 2 years of age and submit proof of completion via email to LPA.
Based on observation and interview, the licensee did not comply with the section cited above where infants acetaminophen medication were on a cart adjacent to the children changing table. The medication was not inaccessible to children. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By COB 12/22/23, Licensee will email LPA Chancellor william.chancellor@dss.ca.gov proof of medications being moved to an area that is always inaccessible to children reach.
Based on observation, interview and record review, the licensee did not comply with the section cited above where fire drill documentation was not available for review during inspection. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By COB 12/22, licensee will email LPA Chancellor an updated docuemnted fire drill log. Licensee stated they understand that dills need to be conducted at least every six months.
Based on observation, interview and record review, the licensee did not comply with the section cited above in three of three infant files reviewed. During nap, there is an adult present but checks are not being logged. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By COB12/22/23, licensee will email LPA Chancellor proof of sleep logs for all children under the age of 24 months. Sleep logs should document 15 minute sleep checks, position of the child, along with labored breathing and or signs of distress when occuring.
Based on interview and record review, the licensee did not comply with the section cited above in one of two staff files reviewed. Licensee stated she believed Mandated reporter training to expire every three years. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By COB 12/22/23, Licensee will email LPA Chancellor an updated mandated reporter certificate for licensee. Licensee current certificate expired 10/3/23.
Based on interview and record review, the licensee did not comply with the section cited above in one of six child files reviewed. During file review, one child files were missing proof of immunizations. Which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: By COB 12/22/23, licensee will email LPA Chancellor proof of immunizations for child 5. Licensee stated they understand proof of immunizations need to be kept in child file and available for review.
Based on interview and record review, the licensee did not comply with the section cited above. Where it was stated, that licensee did not realize a roster needs to be maintained as children are enrolled. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By COB 12/22/23, Licensee will email LPA Chancellor and updated roster provided today by LPA Chancellor. Licensee stated they understand roster needs to be undated as children are enrolled and maintained until they exit. This form must be kept for three years after children leave the program.
Based on interview and record review, the licensee did not comply with the section cited above where current CPR certification could not be located and was not available for review during inspection. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By COB 1/12/23, Licensee will email LPA Chancellor proof of CPR certification for licensee and or assistant 1.
