Based on observation & interview, LPA observed possibly hazardous personal care items in childcare bathroom accessible to children in care, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will make items in childcare bathroom and kitchen area inaccessible to children in care. Licensee will send LPA pictures of bathroom area and kitchen counter with items made inaccessible. Pictures will be sent via text or email by 10/31/25.
Based on interview and record review, S1 and S2 did not have a valid Mandated Reporter training certificate. Therefore, the licensee did not comply with the section cited above in 2 out of 2 staff records reviewed] which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a completed Mandated Reporter certificate for S1 & S2 to LPA via email.
