Resolution: Licensee will ensure that all staff have current mandated reporter training. Licensee's Assistant will complete mandated reporter training by plan of correction (POC ) date. Licensee will provide certificate by poc date.

Resolution: Licensee shall submit proof of complete immunization records for her Assistant . Immunization record shall include TB clearance. Licensee shall submit immunization records to CCL by POC date- 9/4/23.
