Based on observation, interview, record review, the licensee did not comply with the section cited above by not completing the Individual Infant Sleeping Plan [LIC 9227] for C3- C5 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have the parents of C3- C5 complete, sign, and date the Individual Infant Sleeping Plan [LIC 9227] and to submit a copy to CCL by POC due date.
