Based on observation and record review, the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Director agreed to have staff 2- S2 complete her mandate report training and to submit proof of training to CCL by POC due date.

Based on record review, the licensee did not comply with the section cited above by not maintaining her First aid/CPR certification on file and expired on 2/1/2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated her First aid/CPR class is scheduled for 3/22/2025. Licensee agreed to send photo of valid First aid/CPR to CCL by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above by not logging in 15 minute sleep checks for C1, and C2 which are under 2 years of age which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will start conducting 15 minute sleep check for both C1 and C2 and will continue conducting sleep check for all infants under the age of 2 years old. Licensee will submit a signed self-certification on section 102425(D)- Infant safe sleep to CCL by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above by admitting a child under 1 years old without an Individualized infant sleep plan (LIC9227), which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have C1's parents fill the infant sleep plan (Lic9227) out and submit a signed copy to CCL by POC due date.

Based on observation, the licensee did not comply with the section cited above. Licensee had a carbon monoxide detector, but is not functioning, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will purchase a battery for the carbon monoxide detector and send proof to Licensing.

Based on record review, the licensee did not comply with the section cited above. Licensee has not completed the Mandated Reporter training, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will complete the Mandated Reporter training and send certificate to Licensing.

Based on record review, the licensee did not comply with the section cited above. Licensee's CPR/1st Aid card on file expired on 10/2019, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will completed the CPR/1st Aid training and send card to Licensing.

Based on record review, the licensee did not comply with the section cited above. STD 850- Fire clearance granted and license comment states that daycare is limited to converted garage, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit the updated LIC 999A: facility sketch to Licensing, so STD 850 is updated.

Based on observation, the licensee did not comply with the section cited above. LPA observed that there were medication, knives, tools, and a bottle of Clorax disinfectant that was accessible to children, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Deficiency was corrected during today's inspection. Licensee moved tools, medication, and bottle of Clorax disinfectant to the backyard, which is off-limits to the children, and the knives to the upper cabinet that is inaccessible to children.

Based on observation, the licensee did not comply with the section cited above. Upon arrival, LPA observed that there was a child in the living room/family room, which is off-limits. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LPA inspected the living room, family room, and kitchen during today's inspection. Licensee moved anything that poses an immediate risk to children to an inaccessible area. An updated fire clearance is needed.
