Based on record review, the licensee did not comply with the section cited above. Child 1, 4, and 5 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated immunization records for child 1, 4, and 5 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed Lysol wipes, scissors and a shaver in the bathroom and Comet cleaner and vegetable spray under the kitchen sink accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed during visit. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above. LPA observed a saucer chair in the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed an placed it in an off limits area of the home and states she will discard of it.

Resolution: Licensee removed exersaucer from the inside of the home and placed in a storage. Licensee understands she may not have such items in the home.

Based on interview, the licensee did not comply with the section cited above. Licensee and her daughters have not completed Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and her daughters will complete Mandated Reporter training and submit certificate to CCLD by POC date. Training can be found at www.mandatedreporterca.com.

Resolution: Licensee will submit LIC9227 for child 7 to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Child 7 and 8 do not have a Safe Sleep log on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit Safe Sleep log for child 7 and 8 for the week of 09/06/2022-09/09/2022 to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Nayelie Gonzalez does not have fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have Nayelie Gonzalez fingerprint cleared. Licensee understands Nayelie may not be in the home until clearance is obtained.
