Based on observation , the licensee did not comply with the section cited above in backyard had tools laying on the table easily accessible to children in care. The backyard had weeds/brush in backyard in area where children play. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have backyard cleaned and send photos to LPA via email by 11/26/22

Based on observation, the licensee did not comply with the section cited above in licensee had lysol spray can on the back of the toilet in the bathroom utilized by children. Licensee also had toothpast on the side of bathtub accessible to children in care. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed items from being acessible to chlidren in care during time of inspection.

Resolution: Licensee will place posting up as required and send LPA photos of parent board by 11/26/22

Based on observation & interview licensee did not comply with the section cited above in licensee stated she does the drills with the children and they go across the street from facility, no drill log provided at time of inspection, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will provide a copy of the drill log to LPA via email by 11/26/22

Resolution: licensee will document and maintain a record for child #3 and doument the every 15 minute log as required. Licensee will send LPA via email a copy of the log and records for child #3 by 11/26/22

Based on record review, the licensee did not comply with the section cited above, icensee does not have mandated reporter certificate during time of inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will provide a completed certificate to LPA via email by 11/26/22

Based on interview &record review, the licensee did not comply with the section cited above in licensee does not have a CPR/First aide card in hand during time of inspection. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she is a licensed instructor for American Heart Association but they do not give out cards anymore. Licensee will provide card to LPA via email by 11/26/22

Based on observation, the licensee did not comply with the section cited above in no postings other than license was up during time of inspection. which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will post appropriate postings and send a photo of parent board once completed to LPA via email by 11/26/22

Resolution: Licensee stated she has the files ,they will be sent to LPA via email by 11/26/22. chidlren in care are her grandchildren

Resolution: licensee will create and maintain records for all children in care and have readily available for licensing during inspection Licensee will provide records needed for child #3 by 11/26/22 child is licensee's grandchild but was advised records sill need to be maintained as a licensed provider.
