Based on observation the licensee did not comply with the section cited above where during inspection of the backyard, LPA noted several rose bushes with exposed thorns that children would have access to if playing in the backyard. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that the bushes will be made inaccessible and provide proof of such to LPA by POC date.

Based on observation the licensee did not comply with the section cited above where Child One #1 was noted to be asleep in a bassinett instead of transferred to a crib. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that they will conduct in-service training and provide proof of such to LPA by POC date.

Based on record review, Licensee did not have a log indicating 15 minute checks for Child One (C1) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that they will complete a 15-minute check log for C1, and provide proof of such to LPA by POC date.

Based on record review, the licensee did not comply with the section cited above where documentation of an infant sleeping plan was absent. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will develop a LIC9227 for C1 and submit to LPA by POC date.

Based on observation the licensee did not comply with the section cited above inThe bathroom utilized by day care children has cleaning products, in the bathtub easily accessible to children in care. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove items and send a photo to LPA Flores by 10/14/22.

Based on observation &interview, the licensee did not comply with the section cited above in one adult that just move into the household, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's mother is living in the home and has been in the home since 10/01/22. LPA informed Licensee to have her fingerprinted immediately and send LPA Flores information by 11/13/22

Based on record review the licensee did not comply with the section cited above in three out of three staff does not have the mandated reporter certificate. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: licensee stated she took the class but she does not know where the certificate is. Licensee will send LPA Flores completed certificate for herself and S1 via email by 11/13/22

Based on record review the licensee did not comply with the section cited above in CPR/First aide is expired since 07/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will provide LPA Flores via email a copy of completed certificate/card for self and spouse by 11/13/22.

Based on interview & record review, the licensee did not comply with the section cited above in three out of three staff. Licensee does not have staff files for review, which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will create a staff file for each adult in the home and assistants by 11/13/22
